COMMUNITY LIVING OPTIONS INFORMATION PROCESS (CLOIP) INSTRUMENT

To create a CLOIP instrument report open the specific consumer’s chart and click on the “Case Manager/Program Coordinators” tab (pink circle). Then, click on the “Community Living Options” button (red arrow). When the tab is highlighted click on the Add Community Living Option’ button in the yellow header (blue circle).

When the CLOIP window opens enter a date and fill out all the data fields. Suggested text is given in each field but this text can be easily edited if needed.

When all fields are satisfactorily filled click the big blue “Save” button at the bottom of the window.

New CLOIPs will now be listed at the bottom of the consumer chart (blue circle). PDF reports can be created by clicking on “Report” to the right of the specified CLOIP (green arrows).
