The program provider failed to conduct initial and periodic training that ensured staff members and service providers were qualified to deliver services as required by the current needs and characteristics of the individuals to whom they delivered services, including the use of restraint.
Ensuring that providers of direct supports are trained on individual needs is critical to providing adequate care and appropriate interventions! Safety and satisfaction are hugely impacted by Special Needs training, so it’s important to verify that everyone who provides supports receives this necessary information. FS offers two ways to ensure verification of training. First, documentation of those individual needs is available in an easy-to use, easy-to-print format located via the Special Needs link in each consumer’s individual record. (1) Provider application users simply click on the link and fill in the blanks to specify information, tailor supports and interventions, and generate a printable report to use for staff training. Once the direct service provider receives the training and signs the report form, (2) the form can be scanned and uploaded back into the Focused Software system, where it can be stored as a Personnel Checklist item. (By the way, the Personnel Checklist is another recently-released update to the FS system, allowing Program Providers to verify qualifications, training, record expirations, and upload documents to meet certification or customized company requirements.)
The program provider failed to ensure that the service coordinator was provided with a copy of the results of the on-site inspection within five calendar days after completing the inspection.
How do you ensure that on-site inspections are occurring and results are provided to the SC? With Focused Software’s Residential Site Reviews (RSR alternatively called an Environmental Inspection), you’ll never receive this citation again! Residential Site Reviews can be customized, with prompts that you create (or we can provide you with our survey-proven inspection items gathered from Residential Review checklists and from successful providers). RSRs can be completed by designated team members, routed through approvers, and stored in the FS system. RSR reports can also be generated in PDF format, for ease in emailing (with FS-approved encrypted email) or printing! Once the report has been provided to the SC, a copy of verification (email, fax, or even a signed copy) can be uploaded back into FS as an attachment to the original in each person’s individual record. You’ll never lack proof again! The Focused Software provider application will even signal expirations, with notifications for Residential Site Reviews expiring within 30 days.
The program provider failed to enter critical incident data in the DADS data system no later than 30 calendar days after the last day of the month being reported.
This principle has been near the top of the list for years (It was #2 in FY 2016). Incident reports and medication errors must be routed through case managers, nurses, and other approvers, and they seldom arrive unscathed to CARE data entry staff before the 30-day deadline. While no software system can enter Critical Incident Reporting (CIR) data for you, FS is committed to developing a solution which makes compiling the information easier. Focused CIR Reports will enable providers to generate a simple report in seconds which pulls information directly from the Critical Incidents and Medication Errors entered in the FS system. The software will compile information for each contract, with totals for each required entry element in CARE. Look for this new feature to be fully operational this fall!
